HOSPICE RN CASE MANAGER

If you are compassionate, customer service oriented and have critical thinking skills, Lower Valley Hospice and Palliative Care invites you to join our team as a RN Case Manager. Successful candidate will be a member of our interdisciplinary team providing optimal, emotional, physical and spiritual care for terminally ill patients and their families. Conduct evaluations and admit patients to services rendered by Nursing Assistants as needed. This opportunity requires understanding and acceptance of our hospice philosophy.
Must have a RN license to practice professional nursing in Washington (BSN preferred) and a minimum of two years' clinical experience mandatory (previous Hospice experience preferred). Must provide own transportation to and from patients home, i.e., residence, nursing home, assisted living, adult family home or hospital. Ability to use technology (hardware and software). such as computer and cell phone, and Misys software (dta entry for documentation, uploading and downloading). Spanish speaking a plus.
Sucessful candidate will possess a positive, compassionate and empathetic attitude, as well as the ability to communicate effectively and demonstrate cultural sensitivity when working within the service area's communities in primarily Yakima County with an occasional Benton County patient. He/she will endeavor to utilize their teaching, assessment, observation and intervention skills to provide comfort and care and to maximize quality of life to the patients and their families.
